If you have long-term care needs and are interested in the IRIS (Include, Respect, I Self-Direct) program in Wisconsin, first contact your local Aging and Disability Resource Center (ADRC) to receive options counseling to see whether you qualify for the IRIS program in Wisconsin. Experts at the ADRC will establish your functional, financial and non-financial eligibility for Wisconsin's long-term care programs.
Connections has been supporting individuals as an ICA through IRIS since 2015. Connections currently serves persons in the following counties:

We’ll partner with you if you have a physical or developmental disability, or are an older adult and desire to self-direct services under the IRIS program in Wisconsin. We designed our IRIS consultant agency to empower you and support you in making meaningful choices in your life.
Once we receive a referral from the ADRC that you want to work with Connections, our referral specialist will contact you within 24 hours, or the next business day, to welcome you and begin our relationship. Next, you will hear from one of our IRIS consultants within three days to set a date for your initial meeting. You will also receive a welcome mailing that contains additional information about Connections and our consultants.
IRIS consultant services with Connections
Your Connections IRIS consultant will help you understand the program’s requirements, complete the necessary paperwork and answer questions or concerns you may have regarding the IRIS program. Together, we’ll help identify your long-term support needs, desired goals and outcomes. Next, we’ll develop an Individual Services and Support Plan (ISSP) unique to you. Once completed and approved, you’ll be officially enrolled in the IRIS program as a participant.
Along the way, we’ll assist you in maintaining or developing valued social roles and strengthening your community connections. In addition, we’ll work together to strengthen or discover what is important to you, where you live or want to live, what you do and where you go each day — considering the people you love and care about. We believe in the infinite worth of each person and will work to help you thrive.
Meeting with you – wherever you choose
Our consultants are here to assist you – and the people you choose to support you – in identifying your goals and connecting you to support and services to create your personalized plan. We’ll serve as your partner in person at your kitchen table, at a meeting place in the community, over the phone or via email. We encourage you to invite anyone to our meetings who you feel comfortable with and will assist you in making decisions that will improve your life.
The Connections team of consultants knows your community well, the services available to assist you and the requirements to make the services and support a reality. Going forward, we’ll meet or connect with you on a regular basis to support you and to ensure that your services and support are working to meet your goals. And, when you need to make changes, we’ll help you plan for new solutions and ensure your continued eligibility.

We built Connections, an IRIS consultant agency, from the ground up with a group of determined leaders who have personal and professional experience improving and navigating the long-term care system – and who understand it intimately.
Our team members have served as IRIS consultants and have experience as parents of people with disabilities and caregivers of older adults in need of long-term support. As a result, they understand the challenges you face because they live them every day. Our team-based consultant approach helps share these real-life experiences among our members to benefit you.
Connections also draws upon the vast experience of our parent organization, Lutheran Social Services (LSS), which has served Wisconsin and Upper Michigan residents for 130 years. All LSS-affiliated services are offered to state residents regardless of religious affiliation or background.
