Long-Term Care Functional Screener
Carol Smith has dedicated her entire career to human services helping older adults and individuals with disabilities live independent, safe, and fulfilling lives. She is a people person who enjoys getting to know her participants and their supports. Carol is excited to use her knowledge and commitment to helping others as part of the Connections team.
Carol has over 20 years of case management experience helping empower individuals with a variety of abilities and with various challenges. She is excited to put her deep community knowledge to use, to work for IRIS participants.
Prior to joining the Connections IRIS Consultant team, Carol most recently served as a HUD Service Coordinator through Lutheran Social Services. In this position she worked as an advocate for the aging population as well as for those with disabilities, connecting them to resources in the community to help them remain independent in their communities and to help improve their lives.
Before LSS, Carol worked for Care Wisconsin as a Care Manager, where she provided comprehensive case management services to disabled adults and frail elder adults. In that position she completed assessments, functional screens, and member centered services and support plans.
Previously, Carol worked as a case manager in a prevocational setting at Green Valley Enterprises in Beaver Dam teaching employment skills to individuals with a range of abilities and mental health conditions.
Carol graduated from UW Whitewater with a bachelor’s degree in Psychology and a minor in Sociology. She very much enjoys helping others and is passionate about helping people with disabilities. Carol’s favorite past time is spending time with her family and friends. In her spare time she enjoys shopping, watching movies, and biking.
